Our team is composed of many individuals who have your best care in mind. From the Administrator, Director of Nursing, nursing staff, and our Business Office Manager, we are here to listen to any concerns you have about your stay. We encourage you to be open about any clinical issues with our nursing staff and to ask any questions you have about your care or financial obligation. We are here to help you both at pre-admission and during your stay. Please let us know what we can do for you.
The admissions process begins before the resident makes it through the front door. In fact, we are preparing for the stay immediately upon notification that a new resident will be staying with us by developing a plan of care prior to admission. Before you or your family members arrive, we are preparing the room for any medical needs and developing a plan for rehabilitation, recovery, pain management and more.
Upon admission, a member of our team will meet with and your family to gather important information to develop your clinical plan and to ensure proper billing is completed.
Our admissions team will review the documents provided and explain all amenities and services at our nursing and rehabilitation center and answer any questions that you may have. You will also meet with our business office to discuss your financial obligation.
Within 72 hours of admission, our clinical team will meet with you to perform a clinical evaluation that will be used to refine the care plan based on your unique medical needs. This will include information about your diagnoses, medications, recent hospital stays, and current activity level. At this time, all vital information will be recorded. It is at the meeting that our team will consult you and your family to determine objectives for care and the best treatments and activities to achieve these goals.

Members of our clinical staff meet with each resident on an ongoing basis to assess changing clinical and psychosocial needs, lifestyle and discharge plans. Members of our team that may meet with you include our Case Management Coordinator, Activities Director, Rehabilitation Coordinator, Registered Dietitian and Social Services Director.
Together, you, your caregiver and our team will identify needs that need to be accomplished during your stay. Your treatment will be built around these needs in order to achieve discharge goals.
Our center offers many professional services; however, if you would like to use a professional outside our services you have the option to do so. If choosing a professional services provider, please bring their name, address and telephone number with you on the day of admission. All outside service providers must comply with all applicable rules and regulations.
We want your space to be a reflection of you and your home. We encourage you to bring personal items from home as space allows such as pictures, quilt, and small pieces of wooden furniture. Please label all personal belongings with your name. You are welcome to arrange your side of the room as you would like.
Packing for your stay is an important part of getting ready for admission. We recommend that you bring 7-10 days worth of comfortable clothing and shoes that are suitable for rehabilitation activities. You may bring extra clothing if you would like to change throughout the day. Please mark all clothing with written labels or printed in permanent ink that is sewn into the item.
